JOB TITLE: Coordinator
Coordinators are a critical element of the Delta's residential support
system. They directly supervise Senior Instructors, Job Coaches and Instructors.
They generally are responsible for one or two homes at which a total of
4 to six people live. They are responsible for act planning, scheduling
and activities at each home. They also are responsible for participating
in the Individual Support Plan (ISP) process - developing assessments,
attending ISP meetings, developing follow-up documentation and monitoring
the implementation of ISP related instructions and services.
RESPONSIBILITIES:
- Demonstrate an understanding of and commitment to the implementation
of agency standards regarding the individual’s quality of life.
- Demonstrate dignity and respect in all the individual interactions.
- On-site Supervisor.
- Responsible for coverage, when necessary.
- Assist Director in all staff training and identification of training
needs of staff.
- Supervise the implementation of individualized program plans and
work performance of instructors.
- Responsible for ensuring the maintenance of individual records.
- Coordinate domestic up-keep of the program site.
- Maintain all medical charts for the individuals.
- Participate in evaluation of program staff.
- Responsible for accounting of individual's funds and petty cash.
- Train individuals in self-preservation skills. Conduct minimum of
one (1) fire drill per month.
- All other duties as assigned by the Director.
- Attend and participate in all Clinical Meetings.
- Ensure data is recorded.
- Other duties as assigned.
Facilitation of all aspects of program monitoring and operations as outlined
below:
CONSUMER Centered:
- Ensure Annual Review and Behavior Management Programs are up-to-date
and relevant.
- Ensure assessments are conducted on an annual basis and identify services/supports
necessary to promote each individual s personal safety and autonomy.
- Ensure health care examinations are up-to-date and comprehensive.
- Ensure accurate and timely administration of all prescription medications.
- Promote the optimal involvement of each individual in his/her community.
- Promote the awareness and actualization of each individual s human
rights.
- Promote decision-making and involvement in household decisions and
routines for each individual.
- Monitor individual personal funds and insure training to promote
increased financial autonomy.
- Ensure maintenance of all entitlement/benefits.
SUPERVISION RECEIVED BY: Director of Supports
QUALIFICATIONS: B.A. and two years experience working
with people with mental retardation. Good writing and communication skills.
Experience with staff supervision and training. Must have a car, valid
driver s license, and a good driving record.
